Patient Bill of Rights and Responsibilities

At JH Medical Supply LLC, we are committed to protecting and promoting the rights of every patient we serve. The following rights and responsibilities are based on our ACHC-accredited policies and procedures.

Your Rights as a Patient

As a patient of JH Medical Supply, you have the right to:

  • Be fully informed in advance about care and services to be provided, including the disciplines that furnish care and the frequency of visits, as well as any modifications to the plan of service
  • Be informed of your financial responsibilities in advance of service being provided
  • Receive information about the scope of services that the organization will provide and specific limitations on those services
  • Participate in the development and periodic revision of the plan of service
  • Refuse care or treatment after the consequences of refusing care or treatment are fully presented
  • Be informed of patient rights under state law to formulate an Advanced Directive, if applicable
  • Have your property and person treated with respect, consideration, and recognition of patient dignity and individuality
  • Be able to identify visiting personnel members through proper identification
  • Be free from mistreatment, neglect, or verbal, mental, sexual, and physical abuse, including injuries of unknown source, and misappropriation of patient property
  • Voice grievances or complaints regarding treatment or care, lack of respect of property or recommend changes in policy, personnel, or care/service without restraint, interference, coercion, discrimination, or reprisal
  • Have grievances or complaints regarding treatment or care that is or fails to be furnished, or lack of respect of property investigated
  • Confidentiality and privacy of all information contained in the patient record and of Protected Health Information
  • Be advised on the agency's policies and procedures regarding the disclosure of patient records
  • Receive appropriate care without discrimination based on age, race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sex, sexual orientation, or gender identity or expression
  • Be informed of the toll-free number for reporting complaints to the accrediting body: ACHC at 1-855-937-2242

Your Responsibilities as a Patient

As a patient of JH Medical Supply, you are responsible for the following:

  • Provide accurate and complete health information
  • Follow the plan of service recommended by your physician
  • Notify JH Medical Supply of any changes in your condition or needs
  • Treat company personnel with respect and dignity
  • Use equipment safely and as instructed
  • Notify JH Medical Supply promptly if equipment malfunctions
  • Meet financial obligations in a timely manner

Complaint & Grievance Procedure

JH Medical Supply takes all complaints seriously and will investigate and respond within 5 business days. We are committed to resolving any concerns promptly and fairly.

Notice of Privacy Practices

JH Medical Supply LLC is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with notice of our legal duties and privacy practices with respect to your PHI.

How We May Use Your Information

Your PHI may be used or disclosed for the following purposes:

  • Treatment: To provide, coordinate, or manage your healthcare and related services
  • Payment: To obtain payment for healthcare services provided to you, including billing and collections
  • Healthcare Operations: For our internal operations, such as quality assessment, staff training, and business management

Your Privacy Rights

You have the right to:

  • Access and obtain a copy of your health records
  • Request amendments or corrections to your health records
  • Receive an accounting of disclosures of your PHI
  • Request restrictions on certain uses and disclosures of your PHI
  • Request confidential communications of your PHI by alternative means or at alternative locations
  • File a privacy complaint if you believe your privacy rights have been violated

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect the care you receive from JH Medical Supply.

Equipment Warranty Information

  • All products sold or rented by JH Medical Supply carry a 1-year manufacturer's warranty unless otherwise noted.
  • JH Medical Supply will repair or replace, free of charge, Medicare-covered equipment that is under warranty.
  • An owner's manual with warranty information will be provided for all durable medical equipment where available.

Capped Rental Items

Medicare pays a monthly rental fee for certain durable medical equipment for up to 13 months, after which ownership of the equipment transfers to the beneficiary. After ownership transfer, the beneficiary is responsible for service and repair.

Examples include: Hospital beds, wheelchairs, alternating pressure pads, nebulizers, CPAP devices, and patient lifts.

Inexpensive or Routinely Purchased Items

These items can be purchased or rented. If rented, total rental payments cannot exceed the purchase price of the item.

Examples include: Canes, walkers, crutches, commode chairs, and blood glucose monitors.
